Benefit Plans Insurance Providers Eligibility Effective Date
Medical BlueCross-BlueShield or Homeland Healthcare Consistently working full-time (30 hrs+ per week) The 1st of the month following your hire date
Dental MetLife Dental Consistently working full-time (30 hrs+ per week) The 1st of the month following your hire date
Vision EyeMed Vision Consistently working full-time (30 hrs+ per week) The 1st of the month following your hire date
Supplemental Life Hartford Insurance Consistently working full-time (30 hrs+ per week) The 1st of the month following your hire date
Health Savings Account WageWorks Must be enrolled with the BCBS SavingsCare Plan Associates can enroll at anytime once enrolled with the SavingsCare Plan
401(k) Merrill Lynch All Associates who work for  3 consecutive months and 375 hours The 1st of the month following the eligibility mark
Employee Assistance Program CIGNA Behavioral Health Consistently working full-time (30 hrs+ per week) The 1st of the month following your hire date
Employee Stock Purchase Plan AST Equity Plan Solutions All Associates on assignment after 6 months The quarter following the eligibility mark
